We are happy to offer direct billing for patients

  • We ask that patients prearrange their insurance details prior to their appointment between Monday to Friday, during the day from 9:00am – 4:00pm. It helps to prepare your insurance before your scheduled appointment.

  • It is also recommended to arrive 10-15 minutes before your first appointment to fill out your New Patient intake form and additional direct billing forms.

  • Please remember to bring your insurance card and plan member details.
